Accessing Patient Records
Patients may access and request their medical records through the Patient Portal available on our website. After securely logging into the portal, patients can submit an electronic request for their records at any time.
For first-time users, account creation and identity verification may be required to ensure the security and confidentiality of protected health information. Once logged in, patients can conveniently track the status of their request and receive updates through the portal.
Alternatively, patients may contact any of our office locations to request printed copies of their medical records. Our staff will assist in processing the request and preparing the documents in accordance with applicable privacy regulations.
Completed records can be made available for pickup at the front desk. Patients may be required to present a valid photo ID at the time of pickup to ensure proper verification and release of information.
