To help ensure a smooth check-in process, please bring the following items to your appointment or hospital visit.
Required Items
Please bring:
- A valid government-issued photo ID
- Your insurance card(s)
- A list of current medications
- Referral paperwork if applicable
- Emergency contact information
Helpful Medical Information
If available, you may also bring:
- Recent lab results
- Imaging reports
- Previous medical records
- A list of allergies
- Information about prior procedures or surgeries
For New Patients
New patients may be asked to:
- Complete registration paperwork
- Verify insurance information
- Provide medical history information
We recommend arriving 10–15 minutes early for new patient visits.
For Minors
Patients under 18 years old should be accompanied by:
- A parent
- Legal guardian
- Authorized representative
Additional documentation may be requested if guardianship verification is needed.
Medication Information
Please bring:
- Prescription medication lists
- Dosage information
- Pharmacy contact information
Do not stop or change medications unless instructed by your provider.
